    Consumer Direct Care Network-Behavioral Health is seeking an enthusiastic and self-driven individual. This person will be operating in a diverse environment that will be providing numerous services to clients in an efficient and effective manner. The successful candidate will coordinate direct support services, family-based clinical services, and client resources.

    • Design, write, and implement authorizations, assessments, service plans, and crisis plans with families according to Medicaid protocols
    • Complete documentation such as progress, contact notes, and expense reports in an accurate and timely manner
    • Attend educational meetings, medication appointments, and other meetings as needed or required by client needs
    • Meet established quality and productivity goals
    • Facilitate a positive working relationship with staff, and all referral sources
    • Schedule and coordinate all contacts for clients and their families
    • Maintain strong communication with all participants during the delivery of services
    • Ensure strict confidentiality regarding all communication and information related to clients, employees, and all business relations


    • Bachelor’s degree in a human services field - or -
    • Combination of 6 years’ experience and education equivalent to a bachelor's degree
    • Computer proficient - specifically with Outlook and Excel
    • Excellent verbal and written communication skills
    • Must be able to successfully pass a background screening
    • Possess adequate means of transportation
