Partners is currently hiring for an Intake RN - Part Time (28-32 hours per week) Schedule will be every Saturday (9 hrs.), Sunday (9 hrs) and Monday 1/2 day (8 am - 12 noon).

Job Summary:

The RN – Intake coordinates referrals, admissions, medical orders, insurance verification's, and other services for the PHC programs.  This position provides coverage of Weekend Charge, performing client triage for all programs from 8:00 a.m. to 5:00 p.m. on Friday, Saturday, Sunday and rotating holidays. This position serves as a pivotal point in communication with referral sources and service areas to ensure efficient processing of referrals and service delivery.

Essential Duties and Responsibilities:

Receives, screens and appropriately disseminates information related to the coordination of patient care and program effectiveness. Gathers all necessary information and completes data entry of patient demographic information, verifies payer coverage and coordinates logistics and information transfer. Provides the communication link between staff, clients, families, the medical community, other internal departments and other community agencies.

Provides feedback to all referral sources about referral outcomes, services arranged, or reasons for not admitting the client, as appropriate.

While covering Weekend Charge, receives, triage's and disseminates information regarding patient care, service needs and service delivery. Provides clinical, administrative and technical support for the weekend staff of all programs. Acts as the communication link between field staff, management, clients, families and the medical community during business hours on scheduled weekends and holidays.
